Step 1: The Online Form - deadline June 10

Please complete the online yearly enrollment form by clicking here. You must be logged in (click Log In/Out above) using the user name and password sent to you from Jim Heck. If you need help logging in, please contact Jim.

 

Step 2: Activity Fee Statement - one per family - deadline June 10

Please print and fill out the Activity Fee Statement. Even if you have children in more than one division (Lower, Middle, and Upper School), you only need to fill out one of these.

 

Step 3: For each Upper School Student

Download and fill out the following forms:

 

Tuition Statement - deadline August 1

Athletics Medical Release - deadline July 1

Athletics Training Rules and Sportsmanship Pledge - deadline July 1

Bus Reservation Form (only for students who ride the bus) - deadline August 1

Immunization Form (all 9th graders and any student new to St. Andrew's) - deadline July 1

Camp Balcones Springs Release Form (9th grade only) - deadline June 10; please see Camp Balcones Springs Information in Step 4 for additional information

Step 5: Parents' Council Volunteer Signup

Follow this link to read about all of the ways you can have fun volunteering at St. Andrew's.  Once you have read about all of the ways you may participate, you may sign up for committees online.

 

Notes:

The Vision-Hearing-Scoliosis Form is part of the Activity Fee Statement.  It must be completed for all students who are new to the Upper School, and returned by June 10.
